Serious Incidents and Complaints report form

The purpose of this form is to capture information on Serious Incidents (SIs) and / or complaints within, or affecting, Open Mental Health service offers.

Each partner organisation must complete the report monthly, even if no incidents / complaints have occurred.

When completing the form, please do not include any information that may be used to identify service users, individuals within their support network or members of the public. Please be aware that the information you provide may be shared with other members of the alliance including, but not limited to, Partnership Board / Executive Group / Somerset Foundation Trust.
